Employment Forms You will spend a considerable amount of time in the human resources department completing necessary forms for your employment. Come prepared with the personal information or documentation that will be required for a multitude of forms. You will need your Social Security number, emergency contact informa- tion, and other personal information. Examples of employment forms you will need to complete include Form I-9, Form W-4, and benefits forms. Form I-9 Be prepared to complete a Form I-9 Employment Eligibility Verification. The Form I-9 is used to verify an employee’s identity and that he or she is autho- rized to work in the United States. This form is from the US Department of Homeland Security. Figure 13-1 illustrates the portion of the form that shows citizen- ship status. Both citizens and noncitizens are required to complete this form. You must complete and sign the Form I-9 in the presence of an authorized representative of the human resources department. Documentation of your identity must be presented at the time the form is signed. Acceptable documentation commonly used includes a valid driver’s license, state-issued photo ID, or passport. A list of other acceptable documents is listed on the form. The human resources depart- ment will explain this form and answer any questions you may have. Form W-4 You will also complete a Form W-4 Employee’s Withholding Allowance Certificate. A Form W-4 is used by the employer for the information necessary to with- hold the appropriate amount of taxes from your pay- check. Deductions are based on your marital status and the number of dependents you claim, including yourself. Based on your elections, the amounts with- held from your paycheck are forwarded to the appro- priate government agency. Figure 13-2 shows a com- pleted Form W-4. By the end of January, an employer must send each employee a Form W-2 Wage and Tax Statement to use when filing income tax returns. This form summarizes all wages and deductions for the previous year for an individual employee. For example, an employee will receive a Form W-2 in January of 2019 that summarizes all wages and deductions received in 2018.
