Various permission forms should also
be on fi le. These forms should be fi lled
out at the time of the child’s enrollment.
Permission is usually required for such items
as special screening tests or walks around
the neighborhood. To protect children
from being picked up from the center by a
stranger, unauthorized person, or even a
noncustodial parent, a transportation form
should be used. This form should contain
the names, relationship, telephone numbers,
and driver’s license numbers of people who
have permission to transport the child to
and from the center. Staff should check the
identifi cation of anyone other
than the parents who comes to pick up the
child from the program. In the event a parent
wants to add or remove an individual from
the list, a new form needs to be completed.
Privacy Law
The privacy law is designed to protect
children. It states that a child’s records
cannot be given to anyone other than
parents without the parents’ permission.
Give a child’s records only if the parents
have made the request in writing. After
receiving the request, the materials must
be released within 45 days. A form that can
be included in each child’s fi le is shown in
Figure 11.14.
Parents are given unlimited access to
all their child’s records kept by the center.
Included may be screening information,
developmental evaluations, and parent
meeting planning sheets and summaries.
Many teachers share all this information
during parent meetings.
